Indications and Clinical Use

  • Indication:
    • Active immunization against yellow fever in persons 9 months of age or older.
    • Recommended for travelers to or living in areas where yellow fever is officially reported.
    • Required by law for entry to certain countries.
    • Recommended for laboratory personnel exposed to the virus.

Dosage and Administration

  • Recommended Dose:
    • Primary Vaccination: Single 0.5 mL dose administered subcutaneously.
    • Booster Dose: Every 10 years for those at continuous risk.
  • Administration:
    • Reconstitute with the provided diluent (0.9% Sodium Chloride Injection USP).
    • Administer subcutaneously in the deltoid region.
    • Use reconstituted vaccine within 60 minutes.

Contraindications

  • Known hypersensitivity to any component of the vaccine, including egg proteins.
  • Severe allergic reaction after previous administration of yellow fever vaccine.
  • Individuals with immunodeficiency, thymic disorders, or who are on immunosuppressive therapies.
  • Infants under 9 months of age.
  • Pregnant women should avoid unless travel to endemic areas is unavoidable.

Warnings and Precautions

  • General:
    • Evaluate the risk of yellow fever exposure versus vaccine-associated adverse events.
    • Syncope can occur; manage to prevent injury.
  • Special Populations:
    • Pregnant Women: Vaccinate only if necessary; monitor infants born to vaccinated women.
    • Nursing Women: Use with caution; possible transmission through breast milk.
    • Immunocompromised Individuals: Generally contraindicated; assess risk versus benefit.
    • Older Adults (60+): Higher risk of serious adverse events; individual risk assessment recommended.
  • Adverse Reactions:
    • Common: Injection site pain, redness, swelling, fever, headache, myalgia.
    • Rare: Anaphylaxis, yellow fever vaccine-associated viscerotropic and neurotropic disease.

Drug Interactions

  • Vaccine-Drug Interactions: Avoid concurrent use with immunosuppressive treatments.
  • Concomitant Vaccine Administration:
    • Can be administered with measles, mumps, rubella, hepatitis A, hepatitis B, meningococcal, and typhoid vaccines.
    • Separate sites and syringes required.

Storage and Stability

  • Store at 2°C to 8°C. Do not freeze.
  • Use reconstituted vaccine within 60 minutes.

Clinical Trials and Efficacy

  • Immunogenicity:
    • High seroconversion rates (91%-100%) in various studies.
    • 99.3% seroconversion in adults 18 years and older.
  • Safety:
    • Most adverse events are mild to moderate.
    • Serious adverse events are rare but include viscerotropic and neurotropic disease.
