Giant Cell Arteritis (GCA): Clinical Recognition, Diagnosis, and Management

Giant Cell Arteritis (GCA), historically termed temporal arteritis, is a systemic immune-mediated vasculitis affecting medium- and large-sized arteries. Occurring almost exclusively in adults over 50 years of age, GCA presents with cranial symptoms, constitutional signs, and large-vessel manifestations. The most devastating complication is permanent, irreversible monocular or binocular vision loss secondary to ischemic optic neuropathy. Consequently, suspected GCA is a medical emergency requiring rapid clinical recognition and immediate therapeutic intervention.

Time-Critical Clinical Rule

Never Delay Corticosteroid Therapy for Diagnostic Confirmation: When clinical suspicion for GCA is high—especially in the presence of visual disturbances—high-dose systemic glucocorticoids must be initiated immediately. Diagnostic testing (temporal artery biopsy, vascular ultrasound) should be arranged promptly but must never delay therapy, as untreated disease can progress to irreversible blindness within hours.

1. Epidemiology & Vascular Pathophysiology

GCA demonstrates a marked predilection for older adults, with incidence peaking between 70 and 80 years of age (rare before age 50). It disproportionately affects women and individuals of Northern European ancestry, frequently co-occurring with or following Polymyalgia Rheumatica (PMR).

Pathophysiologic Cascade

Granulomatous Pan-Arteritis → Mononuclear / Giant Cell Infiltration & Elastic Lamina Disruption → Intimal Hyperplasia & Wall Remodeling → Luminal Stenosis & Ischemia

Although cranial branches of the external carotid (e.g., temporal, posterior ciliary arteries) are classic targets, GCA is a systemic vasculitis that frequently involves the subclavian, axillary, and vertebral arteries, as well as the thoracic aorta.

2. Clinical Presentation & Diagnostic Clues

(Ocular Emergency)
Arteritic Anterior Ischemic Optic Neuropathy (AAION) via occlusion of posterior ciliary arteries. Includes amaurosis fugax (transient monocular vision loss), diplopia, or sudden painless visual field defects. Transient visual changes are critical warning signs of imminent permanent blindness.
Scalp Tenderness & Abnormal Artery Superficial temporal and parietal artery inflammation. Pain when brushing hair, wearing hats, or resting on a pillow. Examination may reveal a tender, thickened, nodular, or pulseless temporal artery.
Polymyalgia Rheumatica (PMR) Co-Existence Overlapping systemic inflammatory spectrum. Characterized by bilateral shoulder and hip girdle aching with severe morning stiffness. Approximately 15–20% of PMR patients develop GCA, while 40–50% of GCA patients exhibit PMR signs.
Constitutional Symptoms Systemic cytokine release (primarily IL-6 driven). Unexplained low-grade fever, night sweats, fatigue, anorexia, and involuntary weight loss. May be the primary presentation in large-vessel GCA.

3. Diagnostic Evaluation & Imaging Modalities

No single laboratory or imaging result independently establishes or rules out GCA. Diagnosis requires combining clinical evaluation with inflammatory biomarkers, histopathology, and non-invasive vascular imaging.

  • Erythrocyte Sedimentation Rate (ESR) & C-Reactive Protein (CRP): Marked elevation (ESR often >50 mm/h, frequently >100 mm/h; CRP markedly elevated) is characteristic. Normal inflammatory markers make GCA unlikely but do not completely exclude it in high-suspicion clinical scenarios. Thrombocytosis and normocytic anemia are common secondary findings.
  • Temporal Artery Ultrasound: Demonstrates the characteristic “Halo Sign” (hypoechoic circumferential wall edema surrounding the lumen). Highly sensitive and specific in experienced hands; evaluates multiple segments rapidly.
  • Temporal Artery Biopsy (TAB): Historically considered the gold standard. Pathologic examination demonstrates pan-cortical mononuclear or granulomatous infiltration, multinucleated giant cells, and fragmentation of the internal elastic lamina.
    Skip Lesions Warning: Segmental involvement can yield false-negative biopsies. A minimum specimen length of 1.5–2.0 cm is recommended. A negative biopsy does not rule out GCA if clinical suspicion remains high.
  • Large-Vessel Imaging (CTA / MRA / PET-CT): Indicated to evaluate aortic arch involvement, subclavian/axillary stenoses, or isolated large-vessel GCA presenting without cranial symptoms. Standard non-contrast head CT or brain MRI is insufficient for diagnosing arterial vasculitis.

4. Evidence-Based Pharmacotherapy & Management

Subcutaneous or IV adjunctive therapy alongside glucocorticoids. FDA-approved targeted therapy that substantially increases remission rates, reduces relapse frequency, and acts as a potent steroid-sparing agent to limit cumulative glucocorticoid exposure.
Second-Line Steroid-Sparing Agents
(Methotrexate)
Oral/SC Methotrexate evaluated as an alternative adjunctive agent. Considered when Tocilizumab is contraindicated, unavailable, or poorly tolerated, particularly in patients with recurrent relapses or high corticosteroid toxicity risk.

5. Long-Term Complications, Toxicity Monitoring, & Red Flags

A. Glucocorticoid Toxicity & Bone Health Protection

Due to the prolonged duration of corticosteroid therapy (often 12–24 months), proactive toxicity monitoring and prophylactic measures are mandatory:

  • Osteoporosis Prophylaxis: Baseline DEXA scan, daily calcium and vitamin D supplementation, and early initiation of oral or IV bisphosphonate therapy based on fracture risk algorithms.
  • Metabolic & Cardiovascular Surveillance: Screen for steroid-induced hyperglycemia/diabetes, opportunistic infections, systemic hypertension, cataracts, glaucoma, and proximal muscle myopathy.
  • Aspirin Therapy: Routine low-dose aspirin is no longer universally recommended for all GCA patients due to bleeding risks; use should be individualized based on concurrent cardiovascular disease or high-risk vascular features.

B. Large-Vessel Aortic Complications

Up to 10–20% of GCA patients develop thoracic or abdominal aortic aneurysms, aortic dissection, or branch vessel stenoses years after diagnosis. Long-term vascular surveillance via intermittent non-invasive imaging (CTA or MRA) is recommended, particularly in patients with persistent inflammatory markers or large-vessel disease at baseline.

Clinical Practice Pearls

  • Biopsy Timing Post-Steroids: Do not withhold corticosteroids for a biopsy. Histopathologic evidence of GCA remains identifiable on temporal artery biopsy for up to 2–4 weeks after starting high-dose glucocorticoids.
  • Rule Out GCA in Elderly FUO: In patients over 50 presenting with unexplained fever of unknown origin (FUO), persistent high ESR/CRP, or weight loss, evaluate for isolated large-vessel GCA even in the absence of classic temporal headaches.
  • Re-Evaluate Relapses Promptly: A surge in ESR/CRP accompanied by a return of cranial or PMR symptoms during steroid tapering warrants a temporary dose escalation and consideration of steroid-sparing therapy (Tocilizumab).
